Rough idle

I have a 2001 Dodge ram 1500 5.9L work truck. It has 271,603 miles on it I picked this truck up about a year ago. Don't know if the engine is original or not. The problem I'm having is rough idling and idols somewhat bad when driving, but a lot smoother than in idle. It used to be it needed to warm up before it's not running bad at idle then would run fine but the problem has only gotten worse.
